Health Care Fraud Self-Disclosure Protocol

Persons that wish to make a submission to the SDP must use this online submission form. OIG may have additional questions regarding your submission once it has been reviewed, however, this interactive form will guide you through making a complete disclosure. In addition to providing the information required by the form, the disclosing person should also attach a written narrative or other documentation that provides: (1) a complete description of the disclosed conduct, (2) a detailed summary of the disclosing person’s investigation regarding the disclosed conduct, and (3) an estimate of monetary damages to the Federal health care programs as a result of the disclosed conduct, along with a description of the methodology used to determine the monetary damages. The disclosing person should also provide identifying information (i.e., address, organization type, PIN or NPIN, Medicare number, Medicaid number, Tax ID number, and name of the entity’s Medicare Administrative Contractor) for all entities involved in the disclosed conduct and identified in the submission. NOTE: The online submission form allows you to submit attachments AFTER you complete the information required by the form and click “Submit.”

During our review and resolution of these matters, OIG will comply with the Freedom of Information Act (FOIA). Also, the public disclosure of personally identifiable information is restricted by the Privacy Act.

Notifications of someone else’s improper conduct are not appropriate for the Protocol and should be addressed to OIG’s Hotline or submitted through OIG’s Report Fraud form.  

SDP submissions must conform to the requirements outlined in the SDP.  Submissions that do not include all the required information or are not appropriate for resolution under the SDP may be rejected.
